Rose Haven
Rose Haven is at 520 Sanitarium Road in St Helena, holds California licence number 286803790, and is licensed for 32 residents. Its licence was first issued on 5 December 2019, which makes it one of the newer licences in Napa County, and the licensee of record is Rose Haven LLC.
What Does Assisted Living in St Helena CA Really Involve?
Fewer choices than families expect, and a short list that fills up.
Upper Napa Valley is a small market. St Helena, Calistoga, Rutherford and Angwin together hold a modest number of licensed beds, and families who want a parent to stay near their own home are usually choosing between a handful of addresses rather than a dozen. The alternative is a drive down-valley to Napa city, which turns a daily visit into an hour in the car.
At 32 licensed beds, Rose Haven sits in the middle of that landscape. It is far smaller than the hundred-room communities in bigger towns and considerably larger than the six-bed houses dotted around the valley, and that size shapes almost everything about how a day there feels.
Availability Comes First
The practical consequence is availability. A community with 32 beds does not have a wing standing empty, so the first question on the phone is not what the rooms look like. It is whether there is space at all, and if not, how the waiting list works.
Is This a Board and Care or a Larger Community?
Neither, strictly. It sits above the board and care size range and below a full-scale senior living campus.
The National Institute on Aging (2023) draws the line by headcount. Board and care homes, also called residential care facilities or group homes, usually have 20 or fewer residents, with personal care and meals and staff on hand around the clock but no nursing or medical care on site. Assisted living communities, it says, range from as few as 25 residents to 100 or more.
Thirty-two beds land just inside that second category. In practice it tends to mean a staffed kitchen and an activity schedule, which the smallest houses rarely manage, while the staff still know every resident by name, which the largest buildings often lose.
Why the Search Terms Mislead
If you have been searching for board and care in Napa and keep landing on places like this one, that is why. The search term and the licence category do not line up, and the useful question is not what the building is called but how many caregivers are on the floor at two in the morning.
What About Memory Care in Napa?
Memory care is not a separate licence in California, so the word on a brochure is not a guarantee of anything.
Assisted living and memory care can share the same licence category, which is why two communities describing themselves the same way can offer very different things. The NIA (2024) notes that both assisted living facilities and nursing homes may have dedicated dementia units, that some are built to feel homelike, and that families should ask directly whether those units cost more than the rest of the facility.
For a community of this size the questions get specific. Is the building secured, and what happens if a resident tries to leave. How many staff have dementia-specific training, and who provides it. Is there an enclosed outdoor space someone can use unsupervised. And at what point in a dementia would the community say it can no longer meet the need.
The Question That Matters Most
That last answer matters more than the others. A move that has to happen twice is harder on everyone than a move made once to the right place, so it is worth knowing where the ceiling sits before anyone signs.
How Does Medical Care Work for Residents?
It comes from outside. Assisted living provides care, not medical treatment.
The NIA's description of assisted living covers meals, help with personal care, support with medications, housekeeping, laundry and 24-hour supervision. Nursing care and doctor visits are not part of that list. Residents keep their own physicians, and the community's job is to support the plan rather than write it.
In a valley town that creates a transport problem. Getting a frail resident to an appointment means a car, an escort and most of the morning, and appointments that are hard to reach are the ones that get postponed.
Care That Comes to the Resident
Arranging for a physician who visits residents in the community removes that step for routine review, new symptoms and medication changes. It also means someone clinical sees the resident in the setting where they actually live, which tends to produce a more honest picture than a fifteen-minute office visit.
What Does a Licence Issued in 2019 Tell You?
That the record is short, which cuts both ways.
California licenses assisted living as a residential care facility for the elderly, and the public record holds the licensee, the administrator, the licensed capacity, the status, the date the licence began and the inspection and complaint history attached to the address. Licence 286803790 has been running since December 2019.
A newer licence means fewer years of findings to read. There is less evidence of a pattern, good or bad, than you would get from a community licensed in the 1990s, so the record answers fewer questions and the visit has to answer more. Ask the administrator how long the current staff have been there, because turnover tells you things a file cannot.
Look the licence up yourself rather than taking any listing's word for it, including this one. Ownership and administration change, capacity gets adjusted, and the state record is the only version that is current on the day you read it.

What Is the Sanitarium Road Area Like?
Hillside, quiet, and close to a hospital rather than to shops.
Sanitarium Road climbs away from the valley floor east of St Helena, in a wooded pocket of Napa County that feels nothing like the main road through town. It is residential and still, which is pleasant for a resident who sits outside and limiting for one who used to walk to a cafe.
The nearest acute care is close. Adventist Health St. Helena Medical Center stands at 10 Woodland Road in the same 94574 postcode, with emergency services on site. For a family weighing how far a night-time ambulance trip would be, that proximity is one of the stronger arguments for this part of the valley.
What Visitors Will Find Nearby
Downtown St Helena is a short drive for visitors, which matters for the ordinary business of a visit: somewhere to take a parent for lunch, somewhere to pick up what they asked for, somewhere to sit that is not a bedroom.
Contacts and a Reasonable Next Step
Call first, ask about space, and get the physician's report started while you wait for an answer.
Rose Haven can be reached on (707) 963-3748 at 520 Sanitarium Road, St Helena, CA 94574. Ask to visit at a time you choose rather than the slot you are offered, and if you can, go twice: once on a weekday morning and once in the evening, when the staffing pattern looks different.
The LIC 602 is the step families leave too late. A physician has to see the person, complete the form and sign it, and that can take a fortnight to arrange. Starting it in parallel with the tours keeps a room offer from slipping away on paperwork.
If the Short List Runs Out
If the valley's short list is already exhausted, or if you would rather not call ten numbers yourself, a placement agency will know which Napa County communities currently have a bed, which take the level of care your parent needs, and which waiting lists actually move.
